• Critical Access Hospital Fact Sheet - This Fact Sheet is designed to provide additional information about the background and components of the Assessment for CAHs.
• 2026 Assessment Questions & Instructions - Word Document - This document includes all of the questions on the Assessment, as well as instructions for each question. CAHs should use this to review the questions for the Assessment prior to going to the portal to complete their submission. This document can also be used by CAHs to fill in answers as they collect data, before they submit their responses on the Assessment portal.
• 2025 Assessment National Report - This report uses 2025 Assessment data to provide a national overview of some key components of the Assessment.
