The purposes of this project were to assess the feasibility of using the new CMS hospital-wide all-cause unplanned readmission measure to monitor readmissions in CAHs over time, identify successful evidence-based interventions that have been conducted to reduce CAH readmissions, and disseminate information about these interventions to State Flex Programs.
